Theo kết quả báo cáo, nguyên nhân trực tiếp dẫn đến các giai đoạn sưng và đau vú trước khi có kinh nguyệt chính là sự dao động của nồng độ hormone trong cơ thể nữ giới. Nội tiết tố nữ tăng và giảm trong từng chu kỳ kinh nguyệt bình thường, thời điểm không hoàn toàn giống nhau ở mỗi phụ nữ.

Hormone nữ estrogen làm cho các ống dẫn sữa phình to ra và việc sản xuất progesterone khiến các tuyến sữa sưng lên. Chính 2 hiện tượng sinh lý bình thường này khiến cho ngực bị sưng và đau tức. Trong một chu kỳ 28 ngày thì nửa sau chu kỳ (từ ngày 14 – 28) cả estrogen và progesterone đều tăng.
